This is an appointment request form. Filling this out does not guarantee you an appointment. After submitting this form, you should hear back from me within 1 week about scheduling an appointment. A $100 security deposit may be taken at time of booking to secure your appointment. This deposit is non-refundable, but does come off the price of your tattoo upon the completion of the project. If you need to reschedule please notify me at least 48 hours in advance. Cancelling/rescheduling within 48 hours of your appointment will result in a forfeiture of your deposit, at my discretion. At that point if you want to reschedule you will have to leave another $100 deposit. Nothing personal, just business. My prices are based on a rate of $180 per hour and my minimum tattoo cost is $100. My hourly rate is a guideline, not a strict model. I can sometimes flat rate certain projects, like most of my pre-drawn designs. I work hard to maintain accessibility, we can find a way to work together. Nobody under 18 will be tattooed, no exceptions.
